Please fill out the following form to complete your Fall Mini Session booking. Your will be prompted to pay the deposit of $150 at the end of this form, which is due in order to hold your date/time. MINI SESSION INCLUDES: 20 minute session and 20 digital files (digital download). Additional files will be available for purchase. No other purchase required. $150 is due at booking, and $200 is due two weeks before your scheduled shoot.

LOCATION: Location of the fall mini sessions will be at a South Metro Twin Cities location and will be announced within one week before your session.

MODEL RELEASE: I hereby grant Photography By Nealy the right to use and publish images for purpose of display, editorial work, portfolio, advertising, website, Facebook, or publication for promotion purposes.

SHARING IMAGES: When sharing digital files on social media, please alway credit the photographer.

INJURY: Your are agreeing that Photography by Nealy is not liable for any injuries or death before, during, or after your session.

RESCHEDULING: If the weather is not cooperative on the day of your session, Nealy will contact you within two hours before your session time to reschedule.
